Bariatric Seminar

I'm attending the bariatric seminar at my hospital on Monday, August 22nd. I'm super excited. I went to a bariatric seminar back in 2008 at a different hospital. Because of a change in insurance, that previous hospital is no longer covered. I really like the new surgeon I chose. He is experienced in his profession. I hope that he will be at the seminar.I'm also curious as to what information I will receive at the seminar. I know I need to forms for my waived 6 month supervised diet (because my BMI is 50+) and the behavioral exam. Question is, does the hospital provide them or will I need to get them from my insurance. I will find out on Monday!
