The bottle of Bariatric Advantage Ultra Multi vitamins says that doseage is 3 per day......is that all at once or spread out?

That depends.  Is the 3-pill dose in order to get 100% of the RDs of most vitamins or to get 200% (if you are a RNY or DS patient (your profile doesn't say which surgery you had or will have) you should have 200%).

If the 3 pills are for 100%, you can take them all at the same time.  If the 3 pills are for 200%, it would probably be better to spread them out (although I have never seen anything in terms of medical literature that shows that it is necessary to do so).
