This is Mountainview Behavioral& Neuroscience Center.They have several offices in South-Central KY.
My evalualtion took 15 minutes(1 hour in waiting room) he had a form in which he asked me questions.He was looking for any SEVERE mental illness in which you would not be able to make correct judgement in regards to this surgery.
