Went for my psych evaluation on 6/21. This is the only letter holding up my surgeon's office from sending my info the the insurance company.
I spoke w/Dr Primeau on 7/22 and she assured me that the letter would be sent that day. It is now 8/1 and after leaving several messages (no return calls) and the surgeon's office leaving messages and emails, we STILL don't have the letter. And no return phone calls either.
