I took the 567 question test, and another strange word puzzle test then he made me keep and send a week long food diary and 2 exercise plans, one for right after the surgery, and the other a long term one, and then he would send my approval to my surgeon. He ended up sending it before he received my food diary and exercise plans. I don't know why.
