I'm not sure if this was his doing or his staff's but I was already his patient and yet when my surgeons office said all he had to do was write a letter saying he thought I would do well with the WLS, they still charged me $200, would not charge my insurance, and all he did was ask me the questions on the survey and use my answers to fill out the paper. Oh, well, at least it's done and I passed.
