Question:
What do I need to bring to my 1st appointment with the surgeon???
I have my first appontment with a surgeon in two weeks. I will more than likely go with this doctor. What information do I need to bring to my visit? Also, will I need proof of my Gallbladder removal? I had this done about 8 years ago and can't even remember my dr????? :o( — blank first name B. (posted on July 10, 2002)

I agree with prev poster about last 2 yrs of records from pcp, etc, but
also my surgeon wanted a list of meds I was on so I would suggest you get a
print out from pharmacy, also include a letter that your surgeon can submit
to ins co, and a diet history

I just went to my first visit today, and had several papers to fill out! I
had to know current meds )including dosage strength), diet attempts
including how much I lost & how long I was on each, previous surgeries,
with their dates, not who did them, family medical history, & insurance
info of course. I had to repeat this on a couple of forms and was very glad
I wrote it all down ahead of time. I didn't know the dates/loss record from
the diets, but it was in my medical history that I had copied from my PCP's
office. I also had to list any current health problems, ex: sleep apnea,
sob, cp, GERD, Hypertension, etc.
