What do I need to bring to my 1st appointment with the surgeon???

I have my first appontment with a surgeon in two weeks. I will more than likely go with this doctor. What information do I need to bring to my visit? Also, will I need proof of my Gallbladder removal? I had this done about 8 years ago and can't even remember my dr????? :o(
