Question:
I was denied 5 days ago, because of lack of documentation.
Heres is what my deniel says ' unable to certify the surgery as meets BMI, but no documentation of physician supervised weight loss attempts failing repeatedly by the supervising physician. Last military encounter was 1995 WUTG BI attempts since. I tried 2 times in the Air Force at 2 different AFB and some how they conviently lost my records. I don't know if i should fight this or not. Most insurance companies want some documentation to supervised weight loss.
Weight Watchers normally satisfies this condition or Jenny Craig.
Unfortunately, your records appear to be lost. I can think of some sneaky
ways to miraculously find them, but it might not be advisable. There has
to be copies of them somewhere, notes that were taken. Maybe if you can
just remember the name of a doctor you saw at one point and locate that
person, he/she might have their own notes. I know some doctors do that and
some hospitals also. Were you ever seen at a public/private
hospital/doctor other than on base? They should have records. If all else
fails, get on a program right now. Join a weight loss support group that
your physician can document. It might delay the process 3-4 months, but it
will only benefit you. Good luck.
