PHS (healthNet) I need an appeal letter...

PHS (healthNet) sys I should be on the same diet for 3 yrs before they can approve The first letter they requested a phyc evaluation and proof of medically supervised diets... then the second one they said that I need to be on the same diet for at least 3 years before they can approve... Before you start working on an appeal letter, make your insurance company give you a copy of the exact requirements. You can also ask your employer for a copy of the actual policy. Unfortunately, some insurance companies lay down a string of obstacles in the hopes that you will get discouraged and give up. Don't let them get away with it. Once you have the exact requirements in your hands, you can then refute any 'extra' stuff they try to make you do.
