PHS (healthNet) I need an appeal letter...

PHS (healthNet) sys I should be on the same diet for 3 yrs before they can approve The first letter they requested a phyc evaluation and proof of medically supervised diets... then the second one they said that I need to be on the same diet for at least 3 years before they can approve...
