Question:
Does anyone have a sample appeal letter that i can use as a guide
to write a appeal letter for my friend. She was denied surgery by her insurance company and was told she had to write her "own" appeal letter. Appearanly the clinic or drs office dosnt do this for her. I want to help her as much as posible but i am not sure what this appeal letter should consist of. any suggestions? — Deanna Wise (posted on January 23, 2002)

My appeals letter I wrote consisted of the following:
1. Introduction about who I was. I included my weight, height, age, any
comorbidities, and my career.
2. I then retold my diet history including time frame, how much I lost and
regained. As much detail as possible.
3. I then wrote about all the things I was at risk for based on research
from NIH. And other sources available on the web. A good source was ABS.
4. I let them know that I was afraid of death especially at a young age.
5. I also mentioned I knew this was only a tool but nonetheless essential
for your fight with weightloss.
