Does anyone have a sample appeal letter that i can use as a guide

to write a appeal letter for my friend. She was denied surgery by her insurance company and was told she had to write her "own" appeal letter. Appearanly the clinic or drs office dosnt do this for her. I want to help her as much as posible but i am not sure what this appeal letter should consist of. any suggestions?
