Question:
I have to write an appeal letter quickly and need any help I can get.
My plan does cover Bariatric Surgery but you have to meet certain criteria. One of them being having a co-morbidty. I have been recently diagnosed with mild sleep apnea. I need to know what kinds of things to include in my letter. Already this process is taking so long. I was hoping to have the surgery during summer break but at this rate I feel it may not happen. Do I need to include documentation or pictures? They have the referral from my Dr. Thank you to everyone on this site for being so helpful. Any advice would be greatly appreciated. So was your denial because you don't meet the criteria? Can you talk w/
your Dr to make sure you don't have any other comorbidities like high blood
pressure, stress incontinence, gerd (heartburn w/ reflux), chronic back
pain etc... Email me with your denial info and I'll see if I can help ya
get something written up for sure.
Good luck.

Go to YOUR JOURNEY at the top of this page and click on the footstep,
Insurance Troubles. At the bottom of that page is an example appeal
letter. Add and subtract as it pertains to your personal information.
Keep us updated.

Thank you for your response and assistance. My PCP resubmitted the
referral with the new information about the sleep apnea attached. I hope
this works but I am still inclined to write an appeal letter so I can state
my case and have them see me as a real person instead of a number. I can
go into more detail about my experiences instead of just the form they fill
out. Also, they have 14 days to give me a response where the referral can
take longer I think. Thanks again. Kim
