I have to write an appeal letter quickly and need any help I can get.

My plan does cover Bariatric Surgery but you have to meet certain criteria. One of them being having a co-morbidty. I have been recently diagnosed with mild sleep apnea. I need to know what kinds of things to include in my letter. Already this process is taking so long. I was hoping to have the surgery during summer break but at this rate I feel it may not happen. Do I need to include documentation or pictures? They have the referral from my Dr. Thank you to everyone on this site for being so helpful. Any advice would be greatly appreciated.
