Question:
Has anyone had experience with Dr Juarez in PHX?

They haven't responded. In Jan 2001 I called Dr. Juarez for a closed RNY, they referred me to the surgical program at St Lukes. They took ins info over the phone and said it takes a few weeks to hear back. It's the end of Feb now and when I called back they suggest I call my ins myself. I am sure it will be denied at first and think I should just start with a letter of medical neccesity. I feel like the dr's office and St Lukes have dropped the ball and I'm not sure which way to proceed.    — sweet1robin (posted on February 28, 2001)

Did you recieve the packet form from st.lukes with the questioniar in it?? and did you sign all the release forms in the packet and then send that to each and everyone of your doctors? then you enclose your picture and send that to (the completed packet) st.lukes weight management they look it over and so does dr J and then they will contact you..did you also sign up for the mandatory seminare he has because he will not see you till you do that too..good luck in your journey
