Has anyone had experience with Dr Juarez in PHX?

They haven't responded. In Jan 2001 I called Dr. Juarez for a closed RNY, they referred me to the surgical program at St Lukes. They took ins info over the phone and said it takes a few weeks to hear back. It's the end of Feb now and when I called back they suggest I call my ins myself. I am sure it will be denied at first and think I should just start with a letter of medical neccesity. I feel like the dr's office and St Lukes have dropped the ball and I'm not sure which way to proceed.
