Question:
did anyone with bcbs fl ppo get approval w/only 8 mth. supervised diet instead of 12?
I only had thier policy since Aug.2002 so only been on Dr. supervised diet since then only 8 months worth the ins. is asking for 12 i have about 5 co-morbidities and the doctor is writting me a medically neccesary letter ..Did anyone get approved without the full 12 months? any info. would be appriciated Thank You — marie M. BC/BS is just an insurance carrier. Many companies have BC/BS as their
insurance BUT each company sets different restrictions. Company
"A" w/BCBS may cover wls w/o a supervised diet, Company
"B" w/ BCBS may NOT cover wls, w/or w/o supervised diet. Really
it is up to your individual policy with your particular company. Then
again, there may also be a difference betweent he HMO vs PPO plans withing
the same employer and insurance carrier. How vague is that? :( If denied
there is also the apppeals process and working up the chain of command to
see if the restriction has exceptions.
