Question:
SHould I continue to try to persuade my surgeon to bypass more?
I am having open RNY on thursday, Aug 5. My BMI is 47. My surgeon, John Pilcher generally does a proximal bypass of 75 cm for folks with my BMI. It seems that many I have talked to have done better with longer bypass. I have questioned him about this but he assures me that I will lose lots of weight with the proximal. Should I let the subject drop or contine pressing the issue. I know that each surgeon does things differently, but I REALLY want this to work. I feel that this is my last chance. Hello,
This is just my opinion, though learned the hard way. Listen to your
surgeon. If too much intestine is bypassed, it can lead to severe diarrhea,
dumping, and malnutrition. This happened to me and caused me much sickness
and suffering. Your surgeon is the expert, take his advice. Good luck with

My BMI was 39 and I was bypassed 110 cm. I am doing just fine with no
dumping (of course I stick to the food plan) and no gas, no unpleasant side
effects at all. I would worry that 75 cm is not enough at your BMI. I
believe the higher the BMI, the slower the metabolism, and therefore the
more radical of a procedure that would be needed. Some surgeons are still
doing VBG, even though it has been proven to be not as effective as the
RNY, and these surgeons are presenting it to the patient as the best choice
for them, no matter how much they weigh. I believe the surgeon does the
surgery they feel comfortable with. A new procedure has a learning curve
and many surgeons are afraid to jump in head first. Maybe you should shop
around more and get some second opinions from other doctors.
