Question:
My approval letter states only applies to the medical mangement requirements

I received my approval letter today and it states they have determined the surgery is medically necessary. In another paragraph it says only applies to the medical management requirements. Benefit payment is subject to the terms and conditions of your contract. Surgery is supposed to be covered 100% what does this mean?    — kristen H. (posted on May 16, 2001)

sounds to me like you will be covered. i would assume medical management means all the surgical stuff and follow ups. perhaps they are referring to things like a nutritionist and support groups etc since they would not be considered medically necessary.
