My approval letter states only applies to the medical mangement requirements

I received my approval letter today and it states they have determined the surgery is medically necessary. In another paragraph it says only applies to the medical management requirements. Benefit payment is subject to the terms and conditions of your contract. Surgery is supposed to be covered 100% what does this mean?
