Question:
A volunteer to review a diet history.

I am putting together a "diet history" for my dr. to submit. Since I have never done this I was hoping someone might take a look at it and see if it needs more detail or examples before I send it. If you want to email it to me I will look at it for you. I did one for my doc. to submit to insurance. Not a professional by any means but just know what I did and what my insurance waw looking for.

Hi, if you want to see mine, I just got approved in 1 day for surgery with only 1 month of supervised diets but a lot in detail of non, let me know and I can email a copy!
