A volunteer to review a diet history.

I am putting together a "diet history" for my dr. to submit. Since I have never done this I was hoping someone might take a look at it and see if it needs more detail or examples before I send it. I have already been denied once and am hoping to avoid more delays by giving IPA all the answers they need this time. Thank you.
