Question:
I have bcbs/fl/ppo blue choice dont have 12 dr. supr. records
HI have posted on this before about my med. records being thrown away i did find out that in florida they are only required to keep them for 5 yrs.I did find pharmacy records w/ the doctors name on them and that I was on verelan for high blood pressure and also zoloft for depression the pharmacy records are from 1994-1996,and my dad is going to write an notorized letter also stating he went w/ me to this doctor (cause he use to keep an eye on my kids) and that he remembers me being on a 1800 cal. diet a day from him and execise routine.I also have like 3 mth. dr. supr. weight loss and meds. he put me on. do you think that bcbsflppo blue choice will except this + all other diets i have been on in writting.Please let me know what u think thanx so much kelliann — Kelliann S. Kelliann, I have BC BS PPO INDIANA. They required me to have a supervised
weight loss for the "Last" 18 months.My family Dr supplied that
info, But, I would just submit what info you have,explain your prior
records being destroyed. It helps if you put in your own words what diets
and pills you have tried in the past.My Dr just wrote down "LOW
FAT" Diets, diet pills,water pills, and recommended exercise. Surely

I have the same insurance as you in Florida they require the dr
supervised diets I had 14 years of medical records proving the my dr had
me on a diet for my diabetes at this time because I did not go every
month to weigh in --they rejected it I am fighting them--they change the
rules all the time
