I have bcbs/fl/ppo blue choice dont have 12 dr. supr. records

HI have posted on this before about my med. records being thrown away i did find out that in florida they are only required to keep them for 5 yrs.I did find pharmacy records w/ the doctors name on them and that I was on verelan for high blood pressure and also zoloft for depression the pharmacy records are from 1994-1996,and my dad is going to write an notorized letter also stating he went w/ me to this doctor (cause he use to keep an eye on my kids) and that he remembers me being on a 1800 cal. diet a day from him and execise routine.I also have like 3 mth. dr. supr. weight loss and meds. he put me on. do you think that bcbsflppo blue choice will except this + all other diets i have been on in writting.Please let me know what u think thanx so much kelliann
