Question:
Has anyone applied to Unicare in 2004/2005?

I am looking for recent experiences that people have had with Unicare? (Group Policies) Specifically, the response time as well as did they require you to have the psych. evaluation before pretermination or as long as your surgeon required it? Here are my Unicare notes: Called to see if they got the paperwork started and I'm approved! Here's what they required: Referral from pcp Diagnosis code Procedure code Surgeon & hospital name/address/phone TSH levels Psych eval Diet history for 3 years Letter of medical clearance No preauthorization to see surgeon, just for procedure. I wrote a 3 page letter detailing my desire for authorization, including charts and graphs and photos. I was also expeditious at completing my required labs, etc.

I also have unicare and had my surgery in June. they required all the things the previous person who posted indicated. Although I just did a letter stating the different diets I had tried over the years and how my weight impacted my life. Overall they were very easy to work with and I was very pleased at their response time. From start to surgery it only took me about 3 months. Good luck
