Question:
I have Anthem BC/BS POS(Ohio) Qualifications
My understanding is that to qualify I will have to prove an 8 month medically supervised diet. During the past 8 months I was on adipex a couple of time but did not stay on it because of side effects such a nervousness and inablility to sleep at night. So therefore had little results. I get weighed occassionally at my PCP. And we talk from time to time about my weight. Can anyone help me with ideas as to how I can qualify for the medically supervised diet? Can the fact that I saw a Dr and was on adipex be helpful or enough even though I did not remain on it and got no results? .. Thanks in advance for any info. — Sarah H. (posted on February 27, 2003)

Hi, I also have Anthem BCBS ppo of Ohio. I had to get 5 yrs of medical
history (I had used my PCP forever so that part was easy, a few ER trips,
thrapist, psychologist etc). I had WW and a 3 month Dr supervised diet. So
you should be fine. Start a diet NOW while you wait. That way you will have
this documentation if denied. Don't forget to list all your aches and pains
too. I was approved within 3 weeks at 1st try. Good luck. Jamie (surgery
date April 23, 2003).

I also have Anthem. All I had to do is make a list of all the different
diets and pills I did and took for loosing weight and gave them to my
Surgeon. When my doctor turned all of my papers in I was approved within 7
days. I had no problem with the insurance company.
