I have Anthem BC/BS POS(Ohio) Qualifications

My understanding is that to qualify I will have to prove an 8 month medically supervised diet. During the past 8 months I was on adipex a couple of time but did not stay on it because of side effects such a nervousness and inablility to sleep at night. So therefore had little results. I get weighed occassionally at my PCP. And we talk from time to time about my weight. Can anyone help me with ideas as to how I can qualify for the medically supervised diet? Can the fact that I saw a Dr and was on adipex be helpful or enough even though I did not remain on it and got no results? .. Thanks in advance for any info.
