Question:
What do I need to help my approval?

I have been going through voc rehab to have the surgery. I have been approved once but i still have to go through 2 more. My case worker told me today to get anything that would be a positive. I have all my medical records, a letter from my PCP, chiropractor, and gyno. Anyone have any ideas of something else that would help? I don't really have anyone telling me exactly what they need form me to get completely approved so i just want to make sure i get everything possible. thanks everyone    — LaKeAffy (posted on January 22, 2002)

If you can remember all of the Non-supervised diets you were on, make a list of them, maybe the amount of time you were on them and how much weight lost ... as far back as you can remember... it couldn't hurt..

It would be good to write a letter about yourself. Let them know about your struggles and all different attempts at weightloss. A good letter from your PCP is always a winner. However your personal letter adds a picture about yourself. A picture by the way is also a good idea.

I gave my surgeon two lists. The first list contained every effort at weight control I'd ever attempted: diets, nutritional counseling, therapy, exercise, when and for how long, pounds lost, pounds regained, and anyone who supervised or guided the attempt. I listed EVERY diet I could remember and made my best guesses at the other data. I also had copies of my medical records [which the insurance company never wanted after all]. My second list contained every way in which my morbid obesity was affecting my quality of life. I discussed my difficulty walking, how it infringed and limited my personal and professional life, how I ached, joint soreness, being out of breath, etc. I made sure that every effect was included. The doctor addresses my medical co-morbidities. I listed my "quality of life" issues. I got approved in only a few days and the insurance company said my package was "wonderfully complete".
