Question:
Form to take to the Psychiatrist?
Is there a form or what do I say to the Dr. when I go for my evaluation? Iam still trying after 3 weeks to find a Psychiatrist to see me - they are all full or are not taking in new patients - duh! It's a one time visit! It is so frustrating to me. What do I say when I eventually find a DR.? — alsoarty (posted on January 6, 2003)

You may want to advise that you need a psychological assessment and
clearance for bariatric surgery. My doctor's practive requires the Beck
Depression Inventory-II (BDI-II) test be administered. Not sure what it
is, but when I call the psychologist (sp?) office they knew exactly what I
was talking about.

Hi. Maybe you can get the information from your surgeon? My surgeon had
his own Psychiatrist that he used so I had no problem with that. However,
he still gave me a form with what he needed me to do and what the
Psychiatrist needed to do. Good Luck! :)

Hi; I had the same problem finding a doctor..I called my insurance company
and told them I needed the psych eval and they gave me names....about 25
calls later and no luck for the same reasons you mentioned I decided to
call the surgeons office and find out who they reccommend. I got 4
names...I decided if I had to pay the cost out of pocket to expidite things
so be it.....but I then decided to call my insurance company back again,
and found out one of the was on my preferred provider list. It still was a
little bit of a wait, like 4 weeks, but I called once a week to see if
there was a cancellation and there had been , so it was only 1 week
