Form to take to the Psychiatrist?

Is there a form or what do I say to the Dr. when I go for my evaluation? Iam still trying after 3 weeks to find a Psychiatrist to see me - they are all full or are not taking in new patients - duh! It's a one time visit! It is so frustrating to me. What do I say when I eventually find a DR.?
