Question:
Anyone have example letters regarding their health issues and/or diets for surgeon?

My surgeon wants a list of diets I have tried and I wanted to know if anyone had an example letter they gave to their surgeon/insurance company. Also an example letter to insurance company why this surgery should be covered. Thank you.    — Sandra B. (posted on November 25, 2002)

Hello Sandra - I have a sample letter in my profile that I typed up for my primary doctor to use. All she had to do was sign it, when I showed up for my appointment. Feel free to use the "sample" just modify it to suit your situation. Click on my name at the bottom of this response, it'll load my profile for you. Then scroll down, just below the "Aloha" picture, there is a list of key areas in my profile to view. Click on "My PCP Referral letter" ...also, I don't have a copy of my DIET's tried, but if you need help remembering the one's you've tried over time, I have a link for "200 Different Types of Diets".
