Anyone have example letters regarding their health issues and/or diets for surgeon?

My surgeon wants a list of diets I have tried and I wanted to know if anyone had an example letter they gave to their surgeon/insurance company. Also an example letter to insurance company why this surgery should be covered. Thank you.
