Question:
Does anyone have a sample letter for PCP to sign for medical necessity and support??

Hi, Someone sent me an e-mail with a pre-written letter to give your PCP to sign saying that your surgery is medically necessary and that he supports you. My computer crashed and I no longer have it. Does anyone have a sample letter that I could use to go off of? This site has a letter for the patient writing to the PCP about WLS, but I could find nothing about a letter of support. Maria, Check out my profile under the 6 month mark. There is a copy of the letter sent to the insurance company by my PCP. Good Luck.

Hi Maria...I don't actually have a letter, but wanted to ask that if you do get any, if you would also share them with me. I actually asked this question about a week ago and whoever approves the questions sent me an email telling me they weren't approving it because the question has already been posted. I had even replied to them telling them that most of the letters are regarding appeals, not letters from doctors stating medical necessity, but never heard back from her and my question was never posted.
