Does anyone have a sample letter for PCP to sign for medical necessity and support??

Hi, Someone sent me an e-mail with a pre-written letter to give your PCP to sign saying that your surgery is medically necessary and that he supports you. My computer crashed and I no longer have it. Does anyone have a sample letter that I could use to go off of? This site has a letter for the patient writing to the PCP about WLS, but I could find nothing about a letter of support. Thanks!
