Question:
What do I need to send to insurance re: phys suprvsd diet documentation?
My insurance company requires detailed documentation of a 12-week physician supervised diet. I am currently in week #8 of 12-weeks. My doctor asked ME what specifically he needed to send to them. I called them, they said "detailed documentation, not just weight loss." Arrrgh, back to 1st question, what do I need to send to them? What exactly IS detailed documentation? This is my last requirement to meet before they'll approve me, and I do not want to do it again, Can ANYone please help me? I'm desperate at this point! I had to provide my doctor's "clinical notes" of my visits with
him during the 6-month supervised period. Usually, he wrote something like,
"Discussed <insert recommended diet>, patient is compliant with
diet, unable to exercise due to arthritis - weight loss in past month 2
pounds or weight gain in past month 3 pounds." Something along those
lines. Good luck!!!!

I simply went to the doctor's office and got a copy of every thing they had
in writing for the last five years of everything i had ever had done or
been in there to him for. That seem to satisfy my insurance.
