What do I need to send to insurance re: phys suprvsd diet documentation?

My insurance company requires detailed documentation of a 12-week physician supervised diet. I am currently in week #8 of 12-weeks. My doctor asked ME what specifically he needed to send to them. I called them, they said "detailed documentation, not just weight loss." Arrrgh, back to 1st question, what do I need to send to them? What exactly IS detailed documentation? This is my last requirement to meet before they'll approve me, and I do not want to do it again, Can ANYone please help me? I'm desperate at this point! Thanks, very much.
