Question:
Does anyone have any information on Great-West Life & Annuity Insurance Co. approval?

I have the PPO plan with them. My Dr. is Dr. Barzune of Dallas, TX. Any information on the Dr. or Insurance company would be great!!! My pre-determination letter has been sent to the Insurance Company so I'm just waiting and I'll attend a seminar given by Dr. Barzune this Saturday.    — Brenda Kay U. (posted on February 19, 2001)

I also had Great-West, I got approval for my surgery however I had to call them on a daily basis to make sure I would get approved. You need to call the San Jose ofice and speak with your case manager.

Hey Brenda, I just got approval for my surgery thru this ins co. It only took about 3 weeks to get my approval, I did supply them with my med records (which I gathered on my own) and a 19 page report like thing that I put together with my med history, my diet history and a personal letter as to why I feel that the surgery would be beneficial. The main thing was that they would not accept my claim until I had a psych eval. You may want to check into this. Good Luck, Oh by the way I live in so. Ohio.
