Unless you were told to specifically bring anything (your share of payment
maybe?) you don't need anything. I only took my medical cards for them to
copy. That's it! Good luck! Post-op there on 7/16/03.

I had my surgery at St. V's in Carmel. As I recall, all I needed was my
insurance card and photo id. However, you also will need to know the name
and contact info of anyone you want to have listed as emergency contact,
and if you have a living will or a
healthcare power of attorney you should bring that as well. Also, you need
to know what meds you are taking for some of the labs, and any known
allergies to medications. Hope this helps and best of luck to you! The
hospital is a great facility and the bariatric staff are great. Cheers-

I had my surgery at SVCH (Cleveland) in early February. I had to pay my
copay at my preop testing, so you should be done with that - as far as
paperwork - nope, didn't have to bring anything. Otherwise, I brought too
much stuff. Thought I'd want to read, or something, but didn't. I was even
most comfortable in the hospital gown instead of my own nightshirt. Funny,
that. I even brought books on tape that I never listened to. The only thing
I brought that I REALLY used was my toothbrush, hairbrush, and chapstick.
