Seeking Others Who Had Surgery At St. Vincent's Charity Hospital

I will be having surgery at St.vincents and was wondering on the day of my pre-op testing, what all am I supposed to bring to the hospital such as paper work or anything, I live over an hour away and don't want to forget anything !!! Thanks in advance.
