Question:
Is there a form that I can take in for my psych eval?
Ok, I know that I'm asking a question that's been asked before. I just couldn't find it in the library anywhere. I will be seeing someone other than my regular psychiatrist for my evaluation. (Mine's on maternity leave starting Monday.:( ) Anyway, the person I'll be seeing might not be aware of what to ask or whatever. I thought I remember reading a while back ago about their being a website or something where we could get a form that had all the information we would need on there. BTC gave me a paper to give to the psych that outlined everything they will
need to include. I left it in my folder at work. If you still need it
tomorrow, I will post it for you.

they don't need any special questions. they psych eval is just to make sure
you don't have any mental problems and that you are aware that this surgery
is not magic and won't make your life perfect. in other words, that you
have a good understanding of what you are about to do to your body and that
your mental state is ready for such a change.

This is from the paper the surgeon gave me to take to the psych with me:
The evaluation should be by a licensed psychiatrist or psychologist and
should include the use of an "objective assessment instrument"
(The Minnesota Multiphasic Personality Inventory, 2nd edition, (MMPI-2)) or
the Milton Clinical Multiazial Inventory III (MCMI-III)) which should be
completed in a supervised setting. Additionally there should undertake an
evaluative session of 45 - 90 minutes.
I. The report by the psychologist/psychiatrist should include-
Identifying information : Patiens name, sex, age, address, referring
physician.
II. Reason for the referral - The patient was referred for a psycholoical
evaluation to assess the following:
Patient is appropriate for Roux-en-Y Gastric Exclusion Surgery, and Patien
will be post-operative compliant with dietary requirements and procedures.
III. Mental status
IV. Psychological History
Mental history, family history, personal history (Icluding employment),
previous psychological treatment, history of overweight, history of
weightloss, presence of other addictive behaviors
V. Assessment results
Indicate the assessment device used and results. Include a copy of the
patient's profile along with your report. Also, comment on the overall
results of the testing and relate them to the psychological criteria listed
below: i.e. compliance with dietary restrictions, need for additional
psychological support)
VI. Summary and conclusion:
Indicate if the patient meets the following psychological criteria for the
surgery (and include rationale)
* Is this patient motivated to have this surgery? Indicate reasons why.
*Does this patient understand the procedure and aftercare requirements)
*Does this patient have a plan for the aftercare? Indicate your impression
of their social support system.
* Are there any factors which may interfere with compliance with dietary
restrictions and/or post-opterative instructions?
*If the patien does not presently meet the psychological criteria for this
surgery, indicate your recommendations regarding interventions and, if
appropriate, a follow up plan for reassessment.
*Is additonal psychological support needed prior to and/or following this
procedure?
