Is there a form that I can take in for my psych eval?

Ok, I know that I'm asking a question that's been asked before. I just couldn't find it in the library anywhere. I will be seeing someone other than my regular psychiatrist for my evaluation. (Mine's on maternity leave starting Monday.:( ) Anyway, the person I'll be seeing might not be aware of what to ask or whatever. I thought I remember reading a while back ago about their being a website or something where we could get a form that had all the information we would need on there. If someone has any idea where I can find it, I'd really appreciate it. Thanks!
