I have an enlarged intestine just below the reconnection of the pouch and the intestine. It is allowing me to eat more because the food passes directly from my pouch into the enlarged part of the intestines. I have gained 20 pounds and am seeking a revision. I had an RNY 4 years ago and lost 160 pounds. Pretty good except that I started out at 388. I never made it to goal and feel that I should have had a more radical surgery in the beginning, so the doctor said he would shorten my intestines (make it more distal) and try to fix the enlarged part of the intestines at the same time. Anyone had this done and if so - any advice? How much did u lose and was it worth another surgery?    — TBarr (posted on December 6, 2005)

What you described is a failed stoma. If you are considering a revision, I would look into the DS pouch NOT triing to fix the RNY one. This problem has been around for years, according to the Bariatric Associations surgeons have known about it, failed to fix it and ususally refer their patients to diet and excerise. I had mine 'semi' fixed with the lapband but this only works for dense foods nothing else; stuff still goes thru the band into the intestines. If you have a failed stoma; the chances are you'll have it again.. its the opposite of stricture and 'just' happens. Having a DS pouch would prevent that but no guarntee your valve can be restored. As far as 'getting' to goal; NO ONE is guaranteed that. having WLS does not get you to goal. sucess is deemed 65 - 75 % to 85% . The last 20 or so is not a given. Starting out heavier, you might have benefited from being a distal than a prox which could/can have affect. Seriously tho; I would consider the DS over a RNY distal... the chances are your pouch probelm will not be sucessful. good luck
