Question:
I have 2 questions regarding Boeing Regence Blue Shield - Traditional

My first question is, One of the criteria's that is required by my insurance, is to have 1 year of weight loss supervised by my MD. I don't believe I have that information, but in my medical records, I have proof of many diets that I have been on and proof that I have been morbidly obese for over 5 years. Has anyone not had to show the 1 year of weight loss documentation? My second question is what paperwork do I have to have in order to apply to my insurance company?    — GertieMae3 (posted on December 1, 2005)

Janice, I have to do six months and it has to be uninterrupted, monthly visits to my MD. It has been my understanding that the surgeon and/or clinic along with information from you MD are the ones to submit the paper work to obtain approval for the surgery. Also, make sure the doctor that administers the anesthesia is in your insurance network as well. My partner had foot surgery and it turned out that the anesthesiologist was not covered under her insurance. Don't worry about the paper work, just make sure the doctors send it when they are suppose to or have them give it to you and you will send it. NaDine

I have Blue Shield PPO and it was no questions asked. I did however provide to my Dr. all the types of programs and diets including over the counter diet pills I have used. My advice is to get copies of those medical records showing what diet programs you have been on. Then have your Dr. submit the Medical Necessity Letter being sure to list and include your medical records. Hopefully this will help.... Good Luck

If it is in your medical records and i the last two years, they might cover it. Call your insurance company and ask!
