I have 2 questions regarding Boeing Regence Blue Shield - Traditional

My first question is, One of the criteria's that is required by my insurance, is to have 1 year of weight loss supervised by my MD. I don't believe I have that information, but in my medical records, I have proof of many diets that I have been on and proof that I have been morbidly obese for over 5 years. Has anyone not had to show the 1 year of weight loss documentation? My second question is what paperwork do I have to have in order to apply to my insurance company?
