Question:
I have read alot about getting your files ready to submit to the
Insurance company - which I thought the doctor did for you - but is there anything that I need to supply them with to add to my file, or do I submit my own file? I have records that I have been collecting, along with my family history, and the diets and diet drugs I have been on... Please advise as I should have my consultation within a month...Thanks! — Missy L. (posted on May 16, 2003)

The way it worked for me was, I had to get copies of all my medical records
to the surgeon's office and then when I had my first consult, they had all
my medical records and I was examined by the surgeon, he agreed to do my
surgery and then his office submitted to my insurance for approval. The
surgeon's office needs to have all of your medical records, documentation,
and photo's, etc. so if the insurance company ask them for something they
can send it to them. I would make copies of everything you have and mail it
or take it to your surgeon's office before the first consult.

Hi Missy, My surgeon and his insurance lady subbmitted all the the
necessary papers to my insurance company of all my medical records, but
whatever else I had on file or a copy of I still gave to her to send in
with that so my insurance company wouldn't miss a thing. So I suggest you
keep everything you got and give a copy to your surgeon so he can give it
to his insurance person when they go to send in all of the necessary paper
work to your insurance company, that way you don't have to later if they
question anything else that they might need from you. Hope this info helps.
Good luck to you and hope you get approved!!!!
