Insurance company - which I thought the doctor did for you - but is there anything that I need to supply them with to add to my file, or do I submit my own file? I have records that I have been collecting, along with my family history, and the diets and diet drugs I have been on... Please advise as I should have my consultation within a month...Thanks!
