I am a self pay so I'm not sure what form you are referring to really. It
sounds as if the first thing you need to do is to get a new nutritionist
that actually cares and does a good job.
Not exactly sure of what you are asking, sorry,

I'm assuming that you're talking about an "out of country
approval" form? They usually approve you just for a BMI of over 40.
I'm assuming that your doc put your BMI (or even just height and weight) on
your form? Hope this helps.

One of the things I gathered from your question is that you are most
concerned with being approved. This is a normal concern. But I think the Dr
is the most important referall letter. You need to know you are facing a
life changing surgery. This surgery becomes a tool for us. So if your nut
isnt giving you the life changing guidelines and helping you understand
DONT SETTLE FOR THEM. The Nut is just as important of this process as your
Surgeon. Please check into another resource and find a better nut. You
deserve the best start at this, It will be hard enough with out being
guided down the right path. Id like to thank all of the community for all
of your great daily courage. Misty

When I asked my physician to fill out the out of country forms, she gladly
obliged. She only put "obesity" on the form as well. I was flat
out rejected by OHIP! I suggest you have a heart to heart with your doctor.
Find one of the lists on here with ALL the co-morbidities and write down
all the ones that are relevant to your situation. The referral is a VERY
important part of the OHIP approval process. The trouble that I encountered
was just too much for me and I opted to stay in Canada for my surgery. I am
scheduled on January 22, 2009 with Dr. Starr in Toronto. His waiting list
is not very long actually. I understand that all the doctors with the
Humber River Regional Hospital have reduced their waiting times due to a
large influx of money from the government. It may be worth looking into for
you if you do not get approved for OOC. Good luck to you.
