Question:
Name Changes....

I have just recently got married and I have changed my DL but have not changed my last name on my insurance card. I have called my Doctor's office 3 times in the past 2 days and have not gotten a call back. My surgery date is June 14 and my HR department at work has told me I will NOT be able to get a replacement card by my date. Does anyone foresee this being a problem when I get to the hospital on my surgery date?!?!!??    — ReneeKelly (posted on May 24, 2010)

If it were me I would take a copy (not the original) of my marriage certificate etc. to show anyone that needs proof. And just to make certain I would also contact the insurance company AFTER your surgery and reconfirm everything. My ins. company didn't even show that I had already had my surgery. They had the date wrong and was 2 weeks out ahead of me. I called them the day I got home from the hospital and corrected it. I'm sure they would have loved to blame it on me and not pay for my hospital stay so make sure you call them just confirm that their records are correct. Cheers, Tee in Texas

Great advice Tee.It probably wouldn't hurt to send an affidavit to the Surgeon and the insurance company with a copy of your marriage license, stating basically what you just stated here. Your local bank should be able to help you with notarizing it.

I recently got married myself and I kept my last name the same so my husband and I have different last names. I use his insurance. So as long as your medical insurance has your new name in there systems it shouldn't be a problem. I never had to show id or anything when I was using my husbands insurance card that only list his name. oh and shouldn't be a problem if you submitted your marriage certicate to your works HR and the insurance company knows your now marriage. hope this helps or gives you a little relief.
