Name Changes....

I have just recently got married and I have changed my DL but have not changed my last name on my insurance card. I have called my Doctor's office 3 times in the past 2 days and have not gotten a call back. My surgery date is June 14 and my HR department at work has told me I will NOT be able to get a replacement card by my date. Does anyone foresee this being a problem when I get to the hospital on my surgery date?!?!!??
